Work-Study Employee Action Form Request

If you are hiring a student and using Work-Study funding, complete and submit the form below to request a Work-Study Employee Action Form. 

Once the Career Center has 1) received all required information below, and 2) verified the student’s eligibility for Work-Study employment, we will forward the hiring form and processing instructions to you via email.  It can take up to two working days before you receive the Work-Study Employee Action Form.  If you need to hire a student immediately, submit the form below and call the Career Center at 1747 to see if we can expedite the process.

When you receive the Work-Study Employee Action Form, you will need to print, then complete and sign the form, and return the original with signatures to Human Resource Services.  The form is due in Human Resource Services before the student employee begins work (and at least two weeks before the effective date in order for the student to be paid on the first available payday).  If you have questions about the Human Resource Services’ deadlines, please contact that office at extension 1616.  If you are submitting this form on the deadline date (date due to HR), please call the Career Center at 1747 to confirm that we have received the required information and can process the paperwork in time to meet that deadline.  Please inform your student employee when to expect their first pay check.

Date student will begin work -- The student cannot start work prior to you receiving the Work-Study Employee Action Form (WSEAF) from the Career Center. You will then submit the WSEAF to Human Resource Services, and the student must complete the hiring paperwork (I-9, W-4) in Human Resource Services before they begin working.

For a student who is continuing to work into a new fiscal year, a new WSEAF must be completed before that student works in the new fiscal year.

Your Department’s Allocation Number: * This is the number associated with your department’s Work-Study allocation.  This 5-digit number (it may have a hyphen and a 3-digit suffix) is different from your local or appropriated account number.  If you do not know your Allocation #, please contact your Dean or Department Head who should have this information.  Use of this allocation account number should be authorized by your Dean or Department Head to insure money is still available.

Your Department’s Backup Account Number: * The supervisor/department must monitor the balance of the student’s award and your department’s Work-Study allocation.  If either drops to zero, the backup account will be used to pay additional student wages.  To monitor either, refer to your department’s Work-Study report.  Access to this report is given to administrative staff in the office of your Dean or Department Head.  Please contact the Financial Aid Office with questions.
