What You Need to Know About the I-9 Form

The I-9 Form is a federally required document used to verify one’s eligibility to work in the United States.  When being hired, this form must be initiated on or before the first working day in Human Resource Services.  Then, within three business days, appropriate identification (see list below) must be presented.  Identification must be the document itself, a notarized copy of the documentation, or a receipt indicating that the document has been ordered.  For more specific information in I-9 Documentation and current laws, forms and information, please visit http://www.immigration.gov or call Bureau of Citizenship and Immigration Services (BCIS) National Customer Service Center at 1-800-375-5283.  (BCIS - formerly known as the Immigration and Naturalization Service-INS.)

You may provide one piece of identification from List A, OR one piece of identification from both B and C.

US Passport (expired or unexpired)

Unexpired foreign passport, with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorization

Permanent Resident Card (INS Form I-551)

Employment Authorization Document (INS Form I-766)

Unexpired Temporary Resident Card (INS Form I-688)

Unexpired Employment Authorization Card (INS Form I-688A)

Unexpired Employment Authorization Document issued by the INS which contains a photograph (INS Form I-688B)

 

 

  Driver's license or ID card issued by a state or outlying possession of the U.S. provided it contains a photo or info such as name, date of birth, gender, height, eye color, and address

ID card issued by federal, state, or local government agencies or entities provided it contains a photograph or information such as name, date of birth, gender, height, eye color, and address

School ID card w/photograph

Voter's registration card

U.S. Military card/draft record

Military dependent's ID card

U.S. Coast Guard Merchant Mariner Card

Native American tribal document

Driver's license issued by a Canadian government authority

For persons under age 18 unable to present a document listed above:

School record or report card

Clinic/doctor/hospital record

Day-care/nursery school record

U.S. social security card issued by the Social Security Administration (other than a card stating it is not valid for employment)

Certification of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350)

Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal

Native American tribal document

U.S. Citizen ID Card (INS Form I-197)

ID Card for use of Resident Citizen in the United States (INS Form I-179)

Unexpired employment authorization document issued by the INS (other than those listed under List A)
