The purpose of this questionnaire is to enable us to better prepare your interview to meet your needs.  Please print, complete, and fax back (208-426-3437) or drop by the Career Center the form below or click here for the Web Form to submit electronically.  Please attach a copy of your resume or submit your resume by email to career@boisestate.edu noting in the message that your resume is to be used for a practice interview (specify date of interview scheduled).
